Knowledge decays when the source system changes, an exception becomes common, or a decision owner changes without updating the role notes. The risk is highest where the specialist must infer an answer from old examples.
A durable knowledge base needs an owner, source, effective date, review trigger, and escalation instruction. Those fields make it possible to distinguish current guidance from a useful but retired example.
Measure repeated questions and reopened work by topic. The pattern shows where the role needs clearer guidance or where the business should make a policy decision instead of adding more prose.
