Capture repeated error, affected work, evidence, current rule, and consequence. A change request should solve an observable problem.
Route the decision
Name the process owner and any security or legal reviewers. The person logging the change should not approve their own control.
Verify adoption
After the effective date, sample work against the new instruction and record drift. Close only when the owner accepts the evidence.
