Separate signals from conclusions

A late task, quiet account, or unresolved question may be worth review, but it is not proof of dissatisfaction or churn. Use neutral labels and preserve the evidence.

End with decisions

List the owner, requested action, due date, and missing evidence for each issue. The preparation role should make the review faster without making commercial judgments.
